1. Who we are

This website, holosadmit.com, is operated by Trusted Home Health LLC ("we," "us"), the company that builds HolosAdmit™. You can reach us at admin@holosadmit.com.

This policy covers this website only. It does not cover:

  • The HolosAdmit product. How the product handles information is governed by our written agreements with the home health agencies that use it.
  • Trusted Home Health's patient care services. Patients of Trusted Home Health should refer to the agency's Notice of Privacy Practices.

2. Information we collect

Information you give us. When our contact form is active, it asks for your name, agency name, role, work email, phone number (optional), state, and the topic you'd like to discuss. If you email us, we receive whatever you include in your message.

Please do not send patient information. This website is not designed to receive protected health information or any other patient information. Do not include it in the contact form or in email. If we receive it, we will delete it.

Information collected automatically. Like most websites, ours relies on service providers that process basic technical information to deliver the site and keep it working:

  • Our hosting provider processes technical information such as your IP address, browser type, the pages you request, and the date and time of your visit.
  • Our pages load a font from Google Fonts, which receives your IP address when the page loads.
  • Our site platform may receive technical error reports if a page fails to load correctly.

We do not use analytics tools, advertising pixels, or tracking cookies on this website.

3. How we use information

We use the information described above to:

  • respond to your inquiry and schedule conversations you request;
  • communicate with you about HolosAdmit, if you contacted us about it;
  • operate, maintain, and protect this website; and
  • comply with legal obligations.

4. What we don't do

We do not sell your personal information. We do not share it for targeted advertising.

We share information only:

  • with service providers that help us run this website and our email, who process it on our behalf;
  • when required by law, or to protect our rights, our users, or others; and
  • as part of a merger, acquisition, or sale of assets, in which case this policy continues to apply to the information transferred.

6. How long we keep it

We keep contact information for as long as we need it to respond to you and to maintain any business relationship that follows, and we delete it when it is no longer needed, unless we are required to keep it longer.

7. Your choices

You can ask us to access, correct, or delete the personal information we hold about you, or ask us to stop contacting you, by emailing admin@holosadmit.com. We will respond within a reasonable time.

8. Security

We use reasonable measures to protect the information we receive. No website or email transmission is completely secure, so please send us only what you're comfortable sharing, and never patient information.

9. Children

This website is intended for home health professionals and businesses. It is not directed to children, and we do not knowingly collect information from children.

10. Where information is processed

We are based in the United States, and information collected through this website is processed in the United States.

11. Changes to this policy

If we change this policy, we will post the updated version here with a new effective date.
